Your gums are screened and charted, plaque and hardened calculus are removed from above and below the gumline, and you leave with a home routine you can maintain. What a hygiene appointment covers

Hygiene appointments prevent and treat gum disease. They are provided by our dental therapists, working alongside the practice’s dentists.

Dental therapists are registered dental professionals who treat patients either directly or on a dentist’s prescription. Within their competence, a therapist takes your dental and medical history, examines your mouth, completes periodontal examination and charting, uses recognised indices to screen and monitor gum disease, plans treatment and prescribes radiographs where they are needed.

There are firm boundaries on the role and they are worth knowing. A dental therapist does not extract permanent teeth and does not carry out pulp treatment in adult teeth. Tooth whitening can only be provided on a dentist’s prescription. Where something falls outside what a therapist can treat, GDC rules require them to advise you to see a dentist. The scope limits matter more than they sound. An appointment that uncovers a cracked filling or an area of decay is a common enough occurrence, and the point of the boundary is that you get told about it rather than treated around it. That referral runs into routine dental care at our practice without you needing to start again elsewhere.

Why a hygiene appointment is treatment rather than a clean

Hygiene treatment targets a bacterial inflammation of the gums. It is a clinical procedure, not a cosmetic one.

Periodontal assessment comes before instrumentation. A Basic Periodontal Examination divides the mouth into six sextants and scores each one, which is the screening standard set by the British Society of Periodontology. Scores of 3 and 4 indicate detailed six-point pocket charting, recording probing depth and bleeding at six sites on each tooth, along with recession, mobility and furcation involvement. That is how gum disease is found, and none of it forms part of a cosmetic clean.

What the screening looks for is a progression. Plaque builds up on the teeth and irritates the gums.

Gingivitis Reversible

The early stage. The gums are inflamed, but the bone holding the teeth is not yet damaged.

Periodontitis Cannot be reversed, but can be controlled

Left untreated, that inflammation can spread to the supporting bone. Periodontitis cannot be reversed, though it can be controlled and stopped with treatment.

Which of those two you have determines what your appointment involves, and it is how gum disease develops that decides it.

This is also why frequency has no universal answer. A 2018 Cochrane review found that for adults without severe periodontitis who already attend regularly, a routine scale and polish made little or no difference to gum inflammation, pocket depths or quality of life over two to three years. The review did not test what replaces it. What follows clinically is that the appointment earns its place through assessment, through treatment aimed at disease that is actually present, and through changing what happens between visits. Some patients are told they need less frequent treatment than they were expecting. Others are told they need considerably more. Both are recommendations, and both should come with the reasoning attached.

The hygiene appointments we offer

Three appointments are available: a 30 minute scale and polish, a 40 minute air polishing appointment, and a 40 minute maintenance appointment.

Scale and polish 30 minutes, from £75

It begins with a check of your medical history and a Basic Periodontal Examination, with six-point charting where the screening indicates it. Plaque and calculus, the hardened deposit also known as tartar, are then removed from the crown and root surfaces of the teeth above and below the gumline, using ultrasonic and hand instruments, followed by polishing to lift surface staining. You leave with home-care advice specific to what was found, and a recall interval agreed rather than assumed.

Air polishing 40 minutes, from £120

Carried out at this practice with a Woodpecker air polishing unit. It covers everything in the scale and polish appointment and adds an air polishing stage, which directs a controlled stream of air, water and fine powder at the tooth to lift extrinsic staining and disturb bacterial biofilm, including in areas hand instruments reach less easily. It is used alongside instrumentation rather than instead of it, and the extra ten minutes is what that stage takes.

Air polishing tends to suit patients with noticeable staining from tea, coffee, tobacco or red wine, patients with implants, orthodontic appliances or restorations, and patients who find conventional instrumentation uncomfortable. It is not the right starting point for everyone. Untreated periodontitis needs periodontal therapy first, and some medical histories mean a different approach is appropriate, which is part of what the assessment establishes.

Maintenance appointment 40 minutes, from £120

It is for patients who have completed active treatment for periodontitis and moved into the maintenance phase. Full-mouth probing depths are recorded and compared against previous charting to establish whether pockets have responded, sites that have not stabilised are treated, and the recall interval is set against what the charting shows rather than against a standard schedule.

Which appointment suits you is a clinical decision made after assessment rather than something to choose in advance, and the therapist will explain why one is being recommended. Some periodontal treatment runs across more than one appointment, in which case you are told at the assessment how many are likely and what each costs.

How often you should come

There is no single correct interval. It depends on the state of your gums and your risk of disease.

NICE guidance on oral health reviews sets intervals for adults between 3 and 24 months, assigned in three-month steps according to risk and reviewed at each appointment. That guidance covers dental check-ups rather than hygiene visits specifically, but the principle carries: the interval follows your risk, and it changes when your risk changes.

The NHS advises that adults should not go longer than two years without a dental check-up, and that people who smoke or have diabetes may be advised to attend more often so their gums can be monitored closely. For patients who have completed treatment for periodontitis and moved into a maintenance phase, guidance is that full probing depths across the whole mouth are recorded at least once a year.

Booking without seeing a dentist first

You can book directly with our dental therapist without seeing a dentist first.

This is called direct access, and it has applied across the UK since 2013. Dental therapists may carry out their full scope of practice this way, with tooth whitening the single exception, which still requires a dentist’s prescription. The therapist must be satisfied they are trained, competent and indemnified for the treatment, and must advise you to see a dentist if anything falls outside that. In practice it means gum treatment does not have to wait for a dental appointment to be arranged first.

Deepcar Dental Care is a private practice and does not hold an NHS contract, so hygiene fees are private fees. For context on the NHS position: scaling is available on the NHS where a dental professional judges it clinically necessary, and where it is not judged necessary the patient pays for it privately.

What the fee covers is assessment as well as treatment. A Basic Periodontal Examination is carried out at every examination, six-point charting follows where the screening indicates it, and non-surgical periodontal therapy and root surface debridement under local anaesthetic are provided in house rather than referred out. Fees are shown as starting points because the final figure depends on the condition of the surrounding gum and bone, and on the time and materials needed to treat safely. You are told the fee before treatment begins, never after.

Frequently asked questions

Does a scale and polish hurt?

Sensitivity varies between people, and it can be greater where gums have receded or where pockets around the teeth are deeper. Tell the therapist at the start of the appointment if you have found dental treatment difficult before, so the pace can be adjusted to suit you.

Can I have hygiene treatment here if my dentist is elsewhere?

Yes, and you can keep your check-ups where they are. Direct access means no referral is needed, so your existing practice does not have to arrange anything. Tell us who your dentist is when you book.

Will my teeth be whitened at a hygiene appointment?

Not without a dentist’s prescription. GDC rules permit dental therapists to carry out tooth whitening only where a dentist has prescribed it. The appointment does remove surface staining from tea, coffee and tobacco, which changes how teeth look, but stain removal and tooth whitening are different treatments.

My gums bleed when I brush. Should I stop brushing that area?

No. Bleeding gums are usually a sign of inflammation caused by plaque, and stopping cleaning allows more plaque to gather at the gumline. Keep brushing gently with fluoride toothpaste, clean between your teeth daily, and see a dentist if your gums bleed when you brush or eat hard foods.

Is hygiene treatment safe during pregnancy?

Yes, and gum care during pregnancy is worth keeping up. Hormonal changes can make gums more vulnerable to plaque, causing inflammation and bleeding, which is why it is called pregnancy gingivitis. Tell us you are pregnant when you book. NHS dental care is free during pregnancy, though our fees are private.

Do I need healthy gums before cosmetic treatment?

Gum health is assessed before cosmetic treatment is planned, and active inflammation may mean gum treatment comes first. Bonding, veneers and whitening are all planned around stable gum tissue.

How long does a hygiene appointment take?

A scale and polish is a 30 minute appointment. Air polishing and maintenance appointments are each 40 minutes. The therapist will tell you at the assessment which length is appropriate for you and why.
