Black Triangles Between Teeth: Closing Them with Composite

A black triangle is a dark space near the gum where gum does not fill the space below the point where two teeth touch. This page covers telling their causes apart, whether composite bonding can close them, cleanability, technique, black triangles after braces, lifespan and the fee list. The core point: find the cause first, because composite masks the space and does not regrow gum.

What are black triangles between teeth?

Black triangles are small dark gaps at the gum line between teeth, where the gum does not reach the point where two teeth touch. That gum is the interdental papilla, and the point where the teeth meet is the contact point.

The clinical name is an open gingival embrasure. Teeth that do not touch at all have a gap instead, the subject of closing gaps with composite bonding.

What causes black triangles?

Black triangles have three broad causes: lost gum between the teeth, gum recession and bone loss, or the shape and position of the teeth. A 2024 review notes that one person can have several causes at once, so each case needs individual assessment.

CauseWhat is happening
Papilla lossThe gum between the teeth sits below the contact point, linked to how far that point sits above the bone.
Gum recession and bone lossGum disease can destroy bone between teeth. The British Society of Periodontology and Implant Dentistry (BSP) lists gum disease, its treatment, cleaning too firmly and braces among reasons gums recede.
Tooth shapeTriangular crowns tend to touch over a smaller area than squarer ones (2024 review).
Contact point positionIn a 2001 study of adults after orthodontic treatment, short contacts sitting nearer the biting edge were linked with black triangles.
Orthodontic or periodontal historyCommon in adults after orthodontic treatment, the same 2001 study found. The NHS says gum disease can lead to gums shrinking.

Telling the causes apart matters because composite changes only the teeth; where gum disease lies behind the space, the gums need attention first. Our page on receding gums and their causes covers recession itself.

How does the distance from contact point to bone matter?

A 1992 study of 288 sites in 30 patients found the gum present almost every time the contact point was within 5 mm of the bone crest. At 6 mm it was present 56% of the time, and from 7 mm, 27% of the time or less.

A 2003 study of 200 upper front sites in 45 Taiwanese adults, measured on X-rays, found the gum present at 51% of sites at 6 mm and 23% at 7 mm or more, calling the measurement a useful guide for clinical evaluation. A 2006 study of 80 patients examined after gum disease treatment suggests root spacing matters too. These figures describe groups of sites and cannot predict whether gum will fill any one space.

Can black triangles be closed with composite?

Composite can narrow or close a black triangle by building out the sides of the teeth near the gum, so they meet lower down.

It does not regrow the gum: a 2024 review says restorative work masks a black triangle by lengthening the contact, without rebuilding lost gum. The British Society of Periodontology and Implant Dentistry says gum disease must be treated before anything is done to replace or mask receded gums; once gums are healthy, it says, white fillings can disguise the gaps.

What do trials of composite closure show?

The evidence is small and short-term, and a 2014 UK review notes a lack of long-term studies. A 2023 trial in Cairo treated 26 adults aged 35 to 50 with good oral hygiene, no gum disease, no uncontrolled grinding and the two milder grades of gum loss between upper front teeth, comparing two composite techniques. Contact points, tooth form, speech and food trapping were rated successful in every case over 12 months, though tooth form was rated as deviating from normal but acceptable in 6 of 13 cases in one group and 2 of 13 in the other.

Gum scores worsened over the year: at 12 months, plaque and gum bleeding at a level the trial rated acceptable were recorded in 6 of 13 cases in one group, with one further failure, and 9 of 13 in the other. By 6 months most cases in both groups showed staining at the edges, mostly minor and removable, and by 12 months 7 of 13 cases in one group and 4 of 13 in the other had moderate staining. Each group of 13 had one failure by 12 months. The authors say results depend on the operator's skill.

A 2026 trial in China randomised 71 people to composite placed with a dedicated matrix system, conventional layered composite, or no treatment, followed for 6 months. Gum recession progressed in all three groups. Bleeding sites fell with the matrix system but rose from 21.7% to 26.1% with layering, where 8% of restorations failed against none (not statistically significant).

Why does cleanability matter when a black triangle is closed?

A closed black triangle needs to stay easy to clean, since a composite ledge at the gum can hold plaque.

The shape where tooth or composite leaves the gum is the emergence profile. Open black triangles carry risks too: a 2024 review lists food trapping, plaque build-up and a higher risk of root decay, and the NHS says plaque contributes to gum disease and tooth decay. In a small 2016 study, plaque and gum inflammation scores fell a week after overhangs on back-tooth amalgam fillings were removed, though inflammatory markers in gum fluid did not change significantly.

In general, NHS advice on cleaning between teeth includes:

  • interdental brushes or single-tufted brushes instead of floss, especially where there are gaps between the teeth
  • a brush that fits snugly between the teeth
  • avoiding wooden toothpicks, which may damage the gums and could lead to infection

Our page on cleaning between your teeth explains floss and interdental brushes, and how to use them.

How is a black triangle closed with composite?

In general, composite is bonded to the sides of the two teeth beside the black triangle and shaped so they meet nearer the gum. In one technique in a 2023 trial, a matrix strip inserted between the teeth down into the gum crevice shaped the composite, which was then light-cured and polished.

Black triangle closure at Deepcar Dental Care

Deepcar Dental Care offers black triangle closure among its composite bonding treatments, which our cosmetic dentists provide. For composite bonding at the practice, gum disease and decay are treated first, and a mock-up in the mouth previews the planned shape before you decide.

Composite bonding at the practice includes a consultation, photographs, an intraoral scan, a wax-up or digital design, the mock-up, a hygiene visit before treatment, a final polish appointment, a review appointment and an aftercare kit. The technique is chosen case by case; options include freehand sculpting, a silicone key made from a wax-up, injection moulding, layering several shades or a single shade, with a premium composite system selected to suit the case. Composite bonding usually needs no drilling and no numbing injection.

How long does composite bonding last?

For composite bonding, the practice's realistic lifespan figure is around five years. How long yours lasts varies with you, your oral hygiene and your habits.

The practice's 12-month aftercare cover for composite bonding covers repairs, with accidental trauma and infection excluded. Bonding is reviewed 2 weeks after treatment, and a professional re-polish is recommended every 6 months.

With composite bonding, a night guard is required for patients who grind their teeth, and it is charged separately.

Why can black triangles appear after braces?

A 2001 study found black triangles common in adults after orthodontic treatment and linked them to how the teeth sat once treatment had finished.

Measuring 119 of 337 adults photographed after treatment, it found black triangles went with a bone-to-contact distance over 5.5 mm, short contacts nearer the biting edge, roots angled apart and triangular crowns. How rotated or overlapped the two upper front teeth were beforehand showed no statistical link. A 2022 review of five studies of fixed braces, including this one, points to more black triangles after treatment but says a firm conclusion cannot be drawn, and found no consensus on risk factors.

Is black triangle closure priced on the practice's fee list?

Deepcar Dental Care's current fee list does not name black triangle closure, and it does not say whether any of its lines covers it.

When should you speak to your dentist about a black triangle?

Show a new or growing black triangle to a dentist. The British Society of Periodontology and Implant Dentistry says receding gums and gaps between teeth may be a sign of active disease or a consequence of treatment for gum disease.

NHS gum disease guidance says to see a dentist if:

  • your gums bleed when you brush your teeth or eat hard foods
  • your gums are painful and swollen
  • you have bad breath
  • your child has sore, bleeding gums

If you or your child has gum disease and other symptoms, such as very sore and swollen gums, teeth becoming loose or falling out, ulcers or red patches in your mouth, or a lump in your mouth or on your lip, the NHS says to ask for an urgent dentist appointment. With no dentist, or no emergency appointment available, call 111.

Frequently asked questions

Do black triangles mean you have gum disease?

Not always. Tooth shape, where the teeth touch and root spacing can each play a part, while gum disease can cause one by destroying bone between teeth, a 2024 review explains. The British Society of Periodontology and Implant Dentistry says receding gums and gaps may signal active disease or follow gum disease treatment, so have a new one checked.

Is black triangle closure available on the NHS?

Usually not when it is done to improve appearance. NHS guidance says NHS dental treatment covers what you need to keep your mouth and teeth healthy, and that cosmetic treatments are usually private. It also suggests asking the dentist to explain the reasons behind the treatment options suggested.

Can black triangles be closed without composite?

Sometimes. A 2014 UK review groups the options into surgical and non-surgical ones, with restorative work and orthodontic tooth movement among the non-surgical. For the lost gum itself, a 2024 review describes treatment as unpredictable, with several approaches proposed and no gold standard set yet.
